2.  Have your veterinarian fax (425-272-6375) or email your medical history to felinebehavior@drcorigross.vet.

3.  After you submit the contact request form, we will email you a link to the online new patient history form.  If you have any technical issues, please let us know and we will provide you with a Word document instead. 

Once we have the online new patient history form AND the medical record, we can get you on the calendar!  (Current wait time for an appointment - two to three weeks out).
